The photographer is, in fact, a legend amongst rock photographers - Neil "Zloz" Zlozower. The photo itself was taken at John's home (you can see the blue backdrop in a corner on other photos) sometime in the first half of 2006.
This photo appears on cover of two magazines...

It was also given out as a poster with the Total Guitar (UK), December 2006 issue, but only in ASDA, the store chain in UK. Perhaps you can save a search on eBay and look for that poster. I attached a photo of it to this post.
